Carelink

Commonwealth Carelink services, as part of Commonwealth Respite and Carelink Centres across Australia, provide free information about local services and supports available to help people with care needs to continue to live independently in their own home.

Who can we help?

Anyone can use Commonwealth Carelink

  • older Australians
  • people with a disability, chronic health condition or mental illness
  • families and carers
  • professionals, like general practitioners or community support workers
  • carers calling from the south and east country region of South Australia will be linked with the Commonwealth Respite and Carelink Centre, South and East Country, which is a program of Carers SA.  Staff will assess your eligibility and provide information and respite.
  • carers calling from outside the south and east country region of South Australia will be linked with the Commonwealth Respite and Carelink Centre relevant to their location.

 

What kind of help?

We have local knowledge about the services and supports available to support you, your family and the person you are caring for, including:

  • household help, home modification and maintenance
  • personal, nursing and respite care
  • transport and meal services
  • aged, disability and mental health services
  • assessment, including the Aged Care Assessment Program
  • day care and therapy centres
  • special services for dementia
  • continence assistance
  • aged care homes
  • allied health care, like podiatry and physiotherapy
  • carer support services
  • support groups.

We can help you to find out whether you are eligible to receive a service, any costs involved and who to contact to find out more.

We can also give you information about aged care assessment services for access to Community Aged Care Packages or entry into aged care homes.

Freecall 1800 052 222 anywhere in Australia (free call from land-line phones, mobile calls at mobile rates).  Staff can assess your eligibility and link you with services.

You can also ask your GP, case manager, service provider, support worker or a family member to make a referral with your consent.
